PT progress note — Day 13 of SNF stay. Mr. Washington demonstrating significant functional gains since admission for CHF exacerbation. Transfers: sit-to-stand now requires supervision only with use of rolling walker and bed rail — significant improvement from 4-person assist on evaluation. Ambulation: resident ambulated 180 feet x2 in hallway with rolling walker and supervision, SpO2 maintained at 94-95%, no complaints of dyspnea at rest, mild exertion dyspnea resolved within 3 minutes with seated rest. Gait pattern improved, step length more symmetrical. Berg Balance Score 34/56, improved from 18 at evaluation. Resident verbalized understanding of home exercise program reviewed today. Anticipate discharge to home with home health PT within next 1-2 days pending physician and team approval. Goals largely met. Daughter present for home exercise program education and confirmed home environment is single story with grab bars in bathroom.
