PT session note — Day 10 post-admission. Ms. Tanaka participated in 35-minute PT session with fair effort and improving tolerance. Focus: sit-to-stand transfer training and parallel bar ambulation. Transfer now requiring 2-person assist, improved from 4-person assist at initial evaluation — notable functional progress. Resident ambulated 10 feet in parallel bars with min assist x1 and use of right AFO; required moderate verbal cueing for weight-shifting to right lower extremity. Sitting balance improving; able to maintain with supervision for 30 seconds. Resident continues to demonstrate right lower extremity weakness (2+/5). Fatigue noted after 20 minutes of activity. Will continue to progress toward walker ambulation with supervision. Family educated on current functional level and anticipated goals.
