Nursing progress note, day 9 of skilled stay. Resident Sylvia Okafor continues to participate in PT, OT, and SLP per therapy schedule. Tolerating modified diet (IDDSI Level 3 liquids, Level 5 solids) without observed episodes of coughing or aspiration at meals today. Vital signs this shift: BP 134/82, HR 74, RR 16, O2 sat 97% RA, blood glucose 118 mg/dL fasting. Right lateral heel wound reassessed: wound measures 1.9 cm x 1.6 cm, wound bed pink and moist, no exudate, perilesional erythema decreased compared to admission. Resident's mood appears improved; daughter present for afternoon visit and noted resident was more engaged and attempted to initiate words during conversation. Resident continues to require two-person assist for all transfers. No new complaints or acute changes noted. Will continue to monitor. Care conference scheduled for 04/24.
