PT progress note, day 11 of SNF stay. Mr. Fenwick demonstrating meaningful functional improvement since initial evaluation. Transfers now performed with moderate assist x1 from bed and wheelchair; significant improvement from 4-person assist at evaluation. Ambulation initiated: resident ambulated 35 feet in hallway with rolling walker and contact guard assist, O2 sat remained 92% throughout, tolerated without complaint. HR increased from 86 to 104 during activity, returned to baseline within 4 minutes of rest — within cardiac precaution parameters. Resident reports feeling stronger and expressed motivation to return home. Continuing daily PT. Coordination with OT and nursing ongoing for fall prevention and cardiac monitoring. Functional progress supports continued skilled PT services.
