PT progress note — Patricia Delacroix, day 11 of SNF admission. Resident participating in PT sessions consistently this week targeting functional mobility and endurance in the context of congestive heart failure. Sit-to-stand transfers now achieved with mod assist x1, meeting short-term goal. Ambulation progressed to 60 feet with front-wheeled walker and contact guard assist x1; O2 sat maintained at 92-94% throughout with 2-minute seated rest break midway. Resident reporting less dyspnea with ADL tasks per self-report. Therapeutic exercise program completed including lower extremity strengthening in seated position and ankle pumps for edema management. Resident remains motivated and engaged. Continue to progress ambulation distance and endurance toward discharge goal of 150 feet with supervision. Discharge planning communication with social services underway regarding home health PT referral.
